Prepatellar Bursitis

What could cause a knee to become filled with fluid over the kneecap? My father noticed that his knee is very “spongy” but he has no pain. He doesn’t remember injuring it in any way either. It has been like this for about a month and only now is it beginning to hurt a little. He’s 65.
